Dear Dr Stagnaro,
Thank you for your email. The Comment, Editorial, News & Views and Outlooks sections of Nature run commissioned material only and therefore do not accept unsolicited proposals.
There is another option you may wish to consider, if appropriate. This would be to condense your submission to under 300 words as a possible candidate for our Correspondence (‘Letters to the Editor’) section. However, you will first need to check our author guidelines at https://www.nature.com/nature/for-authors/other-subs to determine its suitability and for details of our strict format.
Note that Correspondence is intended for brief comments on topical issues of public and political interest, and for readers’ reactions to informal journalistic material published in Nature; as it is not a peer-reviewed section of the journal, it is not suitable for presenting new data or for technical material.

Dear Nature Editorial Administration,
C/C to All,
your distressing mail is the second photocopied politically correct answer you send me. You did not understand, better said, you do not want to understand the meaning of my first email. I do not want to publish anything at 86 years, in your journals, whose Editors considere T2DM a group of pathologies characterized by “hyperglycaemia”! You and your reviewers are not up to date and you can not understand that the death of Clinical Medicine is the death of Medicine.
There are T2DM Five Stages, Nature Editors and Italian competent Authorities seemingly ignore, bedside diagnosed with a stethoscope from birth. The first three stages are eliminated with Reconstructing Mitochondrial Quantum Therapy, Caramel S., Marchionni M., Stagnaro S. Morinda citrifolia Plays a Central Role in the Primary Prevention of Mitochondrial-dependent Degenerative Disorders. Asian Pac J Cancer Prev. 2015;16(4):1675. http://www.ncbi.nlm.nih.gov/pubmed/25743850[MEDLINE].
Really, based on these paramount, unfortunately overlooked, Medicine discoveries, inexpensive Primary Prevention can be realized rationally on very large scale, stopping the diabetic epidemic. I renew the invitation: I suggest you to fly to Milan next December 3, to attend at my last Lectio Magistralis:

Subject: Is Clinical Medicine dead?
Dear Nature Editors,
after reading the titles of Nature reviews, one thinks that you consider Clinical Medicine dead. In fact, three out of four articles are molecular biological in nature.
I was right when I have stated that “The Death of Clinical Medicine parallels Medicine End”. CMAJ 2008; 178: 1523-1524, 10 June 2008. http://www.cmaj.ca/cgi/eletters/178/12/1523
Let’s look, for instance, at the T2DM Five Stages, whose vulgar, distressing definition is sheared from all Authors, Editors, Reviewers. As a consequence, if a diabetic, under insulin high doses treatment, is normoglycemic, he is no longer diabetic! A scientific Delirium.
